I accepted like 10 missions, and I logged off, when I got back in I didnt have no missions. I still accept a couple of missions but they never get to my account. did I do anything wrong?

You probably double clicked an accept link or two.  What is your game username?

Hi, I've completed the mission "the forlorn ruins" on 1/16/2010 around 4:00am. At that time, the reward was 1 Cashout Card and some MDs (don't remember how much) before Collecting the Reward. After I've clicked "Collect the reward" I've refreshed the Mission page or rather clicked sort by "Status" and the reward became 0 MD & 250 MD. I think something went wrong here. And I think I've got a 10k MD card... not sure and no Cashout card.

The cashout card comes at the end of the entire series.  "The forlorn Ruins" are about the midpoint.  Sort your missions by series, and you should see a new one in the Plague series that comes after Forlorn ruins.

Your mission page should be functional now.  Did you used to be able to get into chat?  have you tried using the /nick command to change your nickname in chat?

I'm sure you are very,very busy but still the mission "the forlorn ruins" reward is 0 MD & 250 MD.

It doesn't matter for me anyhow.
